5/3/2023 EnchantaVaughn Approved (AM)During morning arrival, client was witnessed at his desk on his computer and completing his online assignments. Client said good morning and went back to focus on his work. Client communicated that he finished his entire book and was ready to take the quizzes on the computer. Client needed help with staying on task, due to his poor habits of fidgeting around his seat. Client often lifts up his desk and seat when he feels overwhelmed with his work. His teacher decided to separate him and take his computer away once he showed signs of danger, in order to prevent him from hurting himself and others. Client was given an alternative assignment to work on instead. Client refused to finish his work and kept moving his hands and feet, distracting other students in his class. When confronted, client said he is addicted to it and cannot help when he lifts his desk in the air and stated it helps him relax and stay calm. Client said he couldn't promise it wouldn't happen again and agreed it would be best to come with his counselor to a quieter space to finish his work alone. Iâm the later part of the day, Client began to itch uncontrollably from head to toe as if he was having an allergic reaction. He said he had not put on lotion for months and it made his skin severely dry. Client was escorted to the nurse for a check up. The nurse provided a skin ointment for his skin and asked him to report this to his mom so she could send extra ointment for him to apply when his skin is itchy again. The ointment was extremely helpful and reduced the itchiness right away. Client was a lot more calm and it made him feel much better. He looked moisturized and able to function normally. (PM)QMHP welcomed client to school. QMHP inquired about evening and morning. QMHP asked how client was feeling. QMHP asked if client needed help with anything. QMHP encouraged client to have a good day. QMHP monitored individualized behaviors for acquisition of objectives. QMHP encouraged client to stay in class. QMHP encouraged client to be the example for others. QMHP encouraged client not to follow others he see that are doing wrong. QMHP intervened to assist in explaining behaviors to client and what he needs to work on if he wants to meet his goals and objectives. QMHP reflected with client on his day, reviewing both good and bad experiences. QMHP processed with client, "what would you do different tomorrowâ. QMHP encouraged client to have a good evening. QMHP encouraged client that tomorrow is a new day. QMHP praised client for utilizing coping strategies and being able to get back on track without episode. QMHP processed client positive responses and challenged client to maintain appropriate behaviors throughout the entire week. (EOD)Client had one on one with QMHP. Client stated he got plenty of rest and ate breakfast and was full of energy. Client said he wanted to take a break after finishing his morning work. Client expressed he was ready for todayâs activities. Client shook his head and prepared for teacher instructions. The client was provided with positive feedback as he displayed a realistic understanding of his recurrent pattern of mental illness symptoms. Client said that he would try his best and looked forward to being a leader to his peers. Client has a clear understanding of goals and objectives and agrees to work on making better decisions. Client communicated that he gets frustrated when he does not understand an assignment. Client shared things he enjoyed doing today, as well as, things he disliked doing. Client said that he would be patient and take his time with new activities. Client expressed he wants to get good grades and earn points for participation so that he can be rewarded. Client smiled and promised to have a good rest of the day.
